Outpatient appointments with our certified diabetes educator (CDE) are available for all ages. Education is available for all types of diabetes, insulin pumps, continuous glucose sensors, meal planning and more.
Day Kimball Healthcare's Diabetes Self Management Education and Support (DSMES) Program is an outpatient program provided through our Endocrinology services and is designed for adults with Type 1 or Type 2 diabetes. Day Kimball's DSMES program is led by our registered dietitian (RD) and certified diabetes educator (CDE).
Our program is tailored to your individual needs. We offer individual counseling and group education and support classes to help you learn how to manage your diabetes. You will meet with the diabetes dietitian to review your lab work and discuss your education plan and your goals. Your plan may include a group class series or individual education sessions.
Diabetes education is a recognized part of your diabetes care and is covered by Medicare and most health insurance plans when it is offered through an accredited diabetes education program. Day Kimball's DSMES Program is in pursuit of this recognition through the American Diabetes Association (ADA).
The Diabetes Self Management Education and Support (DSMES) Program includes an individual assessment with our certified diabetes educator (CDE) followed by the option to attend all or some of our 3-week group class series. This interactive program will be held at Day Kimball Hospital in Putnam after attending your individual appointment in Plainfield. The DSMES program is for adults with Type 1 or Type 2 diabetes and will focus on nutrition, exercise, medication management, behavior change and goal setting.
Individual diabetes education and the Diabetes Self Management Education and Support (DSMES) Program is covered by most insurance groups including Medicare. Please confirm with your insurance provider to verify your benefits before scheduling an appointment.
A referral from your primary care practitioner (PCP) is required for both individual education and the DSMES program. Ask your provider for a referral today.

Sarah Balko, RD, CDCES, is a registered dietitian and certified diabetes educator in the Day Kimball Medical Group's endocrinology department at Day Kimball Healthcare. She has been educating patients in improving their health for 20 years. She treats both children and adults, and her specialities include medical nutrition therapy and diabetes education.
