Day Kimball Medical Group (DKMG), a division of Day Kimball Health (DKH), recently announced the successful renewal of its Patient-Centered Medical Home™ (PCMH™) Recognition from the National Committee for Quality Assurance (NCQA). This marks the third consecutive year that DKMG has earned this prestigious acknowledgment which underscores its commitment to delivering high-quality, patient-centered care.
This recognition reflects the best practices and care provided by DKMG’s Internal Medicine, Family Medicine, and Pediatric Medicine practices. By placing patients at the forefront, DKH ensures continuous quality improvement and coordinated care among all healthcare providers. The PCMH program recognizes primary care providers who demonstrate effective coordination and prioritize accessible services, shorter waiting times, enhanced in-person hours, 24/7 telephone or electronic access, and alternative communication methods such as email or telephone care. This approach enables patients to better understand their treatment options and manage chronic conditions fostering a strong partnership with their healthcare providers.
The PCMH model ensures every patient receives comprehensive care from a dedicated team of medical professionals who evaluate and treat their specific healthcare needs. This model strengthens patient relationships, enhances the care experience, and reduces unnecessary emergency room visits and hospitalizations. It aims to provide equitable access to healthcare for all patients, regardless of socioeconomic status, transportation access, or community location.
“We are incredibly proud to have earned NCQA’s PCMH Recognition for the third year in a row,” said DKMG Vice President of Operations Andrew Gerardi. “This achievement is a testament to our team’s unwavering dedication to putting patients first and continuously improving the quality of care we provide. Our commitment to a patient-centered approach ensures that every individual receives personalized, coordinated, and accessible healthcare. We are thankful for our exceptional team of healthcare professionals who work tirelessly to make a meaningful difference in the lives of our patients.”
Kyle Kramer, CEO of Day Kimball Health added, “This recognition from NCQA is a significant accomplishment that highlights our ongoing commitment to excellence in healthcare. It reinforces our mission to provide high-quality, compassionate care to our community. The dedication and hard work of our medical professionals are the driving force behind this achievement, and we will continue to strive for even higher standards in patient care.”
The PCMH Recognition program involves a rigorous annual reporting process, completed in March after months of data submission preparation. DKMG’s outpatient locations must meet comprehensive performance benchmarks in patient care, including effective care planning, coordination, and access to care, as well as measuring performance and quality.
