Occupational therapy focuses on the treatment of conditions that limit an individual’s ability to perform the tasks of daily living, work and leisure activities. The goal is to help the patient function as independently as possible through the use of physical treatment techniques as well as therapeutic changes or adaptations to their skills and environment.

Our highly qualified and experienced occupational therapists and Certified Occupational Therapy Assistants (COTAs) will guide you in the development or relearning of skills needed to perform the activities of daily living and occupational tasks. Our therapists also offer specialized care for the hand and arm. The therapist will utilize exercises and manual techniques as well as splinting to improve function.

Occupational therapy may be used alone or in combination with physical therapy or other therapies as part of a comprehensive rehabilitation plan. Your therapist will work with the rest of your care team, including your primary care physician, occupational health physicians, surgeons and other specialty care providers.

Occupational therapy may be used to treat injuries or illnesses such as:

  • upper extremity orthopedic conditions such as fractures, tendonitis, hand trauma and post-operative repair
  • arthritis
  • neurological impairments such as stroke, head injury or multiple sclerosis (MS)
  • chronic pain syndromes such as fibromyalgia and chronic fatigue
  • lymphedema and shoulder/elbow/hand limitations resulting from breast cancer and/or breast cancer surgery


Information and Forms for New Patients

Ask your physician for a referral to DKH Occupational Therapy services. Patients may be treated as an inpatient, outpatient or in-home depending on the particular needs and treatment plan. During the initial evaluation, your therapist will ask questions relating to the onset and nature of your illness, injury or condition, previous and current functional level, the nature of any past and current medical history, and your goals for treatment. These questions, and others, will enable us to plan and coordinate your treatment for optimal results.

  • Be sure to bring your physician referral and all insurance information with you.
  • Bring a complete list of all medications (prescription and over-the-counter), vitamins, and/or herbal supplements you are currently taking.
  • Please arrive at least 15 minutes early for your initial appointment to complete all necessary paperwork.
  • Or, you may wish to fill out a Physical Medicine Outpatient Registration Form and Day Kimball Medical Group Patient Forms and Information to fill out at home and bring with you to your first appointment.


Billing and Payment Information

Billing for occupational therapy services delivered at a Day Kimball Healthcare location will be submitted to your insurance by Day Kimball Hospital. Billing for services provided at home will be submitted to your insurance by Day Kimball HomeCare. Please check with your insurance carrier for details about your specific coverage, copays, deductibles and any prior authorization requirements.
